Upline Release Instructions To Agent 

Option 1.) Immediate Upline Release Request

To perform a release please follow these instructions below. 

  1. Contact your current upline and request a Immediate Upline Release Request 
  2. Make this request for all desired carriers 
  3. If the upline agrees signed release forms will be sent to the carriers for an immediate release
  4. Sign the release form and follow up with the upline and carriers for release status
  5. Once released contact your agent rep 
  6. Send a copy of the signed release forms to agents@mystatehealth.com 

This is the fastest option for a release. Once done we can finish your onboarding!

Notice: If you do not know who your current upline is you may contact your carriers to verify their name and contact details. 

Notice: If they decide to not release you we can do a self release which may take 30-90 days. 

Option 2.) Self Release – Notice of Intent to Transfer

In the event an upline will not release you, agents may perform a self release with each carrier.

Once you submit a self release request to each carrier will process this request within 30-90 days.

Once Completed! you will be able to continue on with onboarding and recontract with carriers.

Carrier Release Instructions! 

Below are self release instructions for each carrier – complete each to begin your self release. 

  • Attached you will find several carrier release forms. 
  • Complete the following steps below for each carrier – submit attached forms for each carrier. 
  • You should receive release notices via your upline or carriers within the next 30-90 days. 
  • Follow up with the carrier periodically during the release process.

United Healthcare – Instructions 

Steps

1.) Email the following information to the carrier and following emails – in the same conversation.

2.) Include your agent information in the blanks. 

Email Subject Line:  “Intent to Move” 

Email Body: 

I ___________, I would like to transfer my appointment with United Healthcare to Wealth Alliance Group, writing ID 6568473

My UHC Party ID is __________, UHC Writing ID is __________ My NPN is ___________.

Humana – Instructions 

Steps

1.) Email the following information to the carrier and following emails – in the same conversation

agentsupport@humana.com, agents@mystatehealth.com and contracting@wealthalliancegrp.com

2.) Include your agent information in the blanks. 

3.) Complete and include the attached Humana MGA Assignment Form – Click Here

  • Note: this form must be signed by your new upline.
Email Subject Line: “Release Request Notice of Intent to Transfer” 

Email Body: 

I __________, would like to transfer to the new upline on the attached and signed Humana MGA Assignment Form. The reason for the transfer is for better opportunity for growth and support. 

Include the attached signed Humana MGA Assignment Form.

Wellcare – Instructions

Steps 

Complete the following steps in your Wellcare carrier portal. 

  • Login to https://desktop.pingone.com/ 
  • Click centene workbench 
  • Click my account 
  • Click my hierarchy info 
  • Click the “change” button and enter NPN 21275346 (My State Health LLC)
  • We will be Notified to Approve Transfer
  • You will be transferred in 7 to 10 days

Aetna – Instructions

Steps

1.) Email your full name and NPN to agents@mystatehealth.com 

    Email Subject Line: “Aetna Release Request With Agent Information” 

2.) We will then email you a contracting link for Aetna with the proper upline details. 

3.) During contracting you will need to fill out and sign the top half of the following form. 

4.) Upload the signed form to the Aetna Contracting Portal

  • Located in the same email contracting link or in the Aetna producer world.

Steps

3.)Complete and include the attached Cigna Release Request Form

We will then notify the carrier and provide contracting instructions.
